Dr. Aldo Guerra has stated that to be successful with breast augmentation you must fulfill, as best as possible, the patient’s desires and wishes. In order to accomplish this, Dr. Aldo has developed a liberal philosophy with regards to using different breast implant sizes and breast implant shapes. Silicone breast implants and saline breast implants samples are available in a range of sizes for patients to inspect at the consultation. Depending on the patient’s desired size, we have in our office, both high profile breast implants and moderate plus profile breast implants. Breast implants start at a size of 120 cc and go up to 880 cc.
Dr. Aldo’s philosophy for breast augmentation also includes a belief that a breast enlargement should be proportional to the rest of the body. We have experienced that proportional breast enlargement usually gives the best long-term results and happiest patients. Breast augmentation surgery can be performed anytime during a woman’s lifetime. Women with children often undergo breast augmentation at our practice. There is no need to wait until after pregnancy to have breast enlargement and it's a popular procedure for women that haven’t had any kids yet. Breast implants support the breast better and, in most cases, future pregnancy patients experience less sagging.
Many patients choose to combine other plastic surgery procedures during a breast augmentation. The tummy tuck and liposuction are the most popular. The breasts can also be lifted with a breast lift at the same time as a breast augmentation. Below the muscle breast implant placement.
Patients with small breasts, breast implants can be seen under the tissues and in some situations, wrinkling and rippling can be seen at the breast surface when implants are placed under the breast gland. Therefore, placement of implants under the pectoral muscles is recommended in patients with under developed breast tissue to avoid these situations in our practice. In general, breast implants placed under the pectoral muscle are supported better and experience less sagging with weight changes, pregnancy, and aging. In our experience, recovery from sub-pectoral breast augmentation is similar to sub-glandular breast augmentation. There are some advantages of placing breast implants under the pectoral muscle:
- More tissue coverage resulting in less implant visibility.
- A more natural-looking breast profile.
- Reduced rate of capsular contracture or scar contracture.
- An improved mammogram visualization.
Above the muscle breast implant placement.
Breast implants can also be placed under the breast gland. Patients with adequate breast tissue can be good candidates for this procedure. Possible advantages of sub-glandular breast augmentation include:
- Creation of an unnaturally high, rounded and/or bubbly-looking breast.
- For patients with mild sagginess, sub-glandular breast augmentation may help naturally lift the breast.
Sub-glandular breast augmentation can create bubbly-looking breasts that resemble those of many models, Hollywood personalities and other celebrities. For patients who wish to have this look, this is the recommended procedure. After surgery, sub-glandular breast implant placement patients are at greater risk of having scar contracture around the implant and may experience more sagging with aging.
Breast augmentation risks.
All plastic surgery carries certain risks associated with anesthesia, scarring, and suturing. Breast augmentation, has certain additional risks due to the nature of the silicone breast implants and saline breast implants.
Common breast augmentation risks include:
- Breast pain
- Potentially permanent changes in nipple or breast sensation
- Scar tissue that distorts the shape of the breast implant
- Implant leakage or rupture
- Being unhappy with your results, despite adequate surgery
- Need for additional breast surgery
There are other less common risks that are discussed in a consultation.

Breast augmentation incisions.
Breast augmentation incisions are made to be as inconspicuous as possible, usually under the breast, around the lower part of the nipple or the armpit. All breast augmentation skin incisions are noticeable, although some are less noticeable than others. Some patients are better candidates for one type of incision versus another, because of skin composition, texture and color and we highly recommend a consultation with Dr. Aldo Guerra to determine your options.

Under the breast crease incisions.
When the breast augmentation incision is made below the breast, in the crease, there are several advantages;
- Incisions placed in breast creases, when fully healed, will look like part of the crease, becoming almost invisible to the human eye. This principle is used in almost every plastic surgery procedure to help camouflage incisions as best as possible,
- this incision allows immediate access to the pectoral muscle for under the muscle implant placement. The surgeon can choose to place the breast implant either below or above the muscle creating minimal swelling and disturbance to the tissues. This results into a smoother and more comfortable recovery,
- normal breast tissue begins above the lower breast crease incision. An incision placed here allows for the best anatomical placement of the breast implant. The risks of the breast implant ending up too high in the chest, which gives patients an unnatural, operated-on look, are minimized,
- because the surgeon is closest to the breast using this incision, the
plastic surgeon has greater control and command over the breast tissue. When the plastic surgeon has greater control, results are much more predictable.
The armpit incisions.
Armpit incision or transaxillary incision starts high on the lateral chest wall and extends through the tissues to reach the inferior breast crease. While some patients believe that this procedure results in augmentation without scars, in reality, this incision can be quite noticeable after breast surgery. In some patients, the incisions can prevent the individual from shaving the armpit area where the incision becomes tender. Additionally, because surgery takes places through the tissues and starts so far away from the ultimate location of the breast implant, there is usually more swelling and discomfort after surgery.
The nipple incision.
Nipple incision is used in patients with a very distinct border between the nipple and the rest of the breast. In these patients, carefully placed incisions become almost invisible after the healing portion of the recovery is complete. After the nipple incision is made, a tunnel is created down to the level of the breast muscle. At this point, your plastic surgeon can choose to create a pocket either above the muscle or below the muscle to place the implant. Because the procedure involves surgery through the breast tissues, there is often more swelling when using this nipple incision.
Cristina's Story - Breast Augmentation
The belly button incision.
The belly button incision can be used to place breast implants under the breast gland. Placement under the pectoral muscle is usually not possible with this approach. The incision is made in the belly button and surgery is carried forward towards the breasts. Through a long, specially designed tube, the plastic surgeon works to create a space under the breast gland. Once the pocket is completed, a breast implant is placed under each breast, using the same tube. This approach can be an option for some patients; however, the procedure requires such specialized equipment that the overall costs of the procedure are increased and we currently do not offer the belly button incision as an option at Guerra Plastic Surgery Center.
Breast implant type, size and shape.
Multiple breast implant options exist for our patients. All modern breast implants currently available have a silicone shell on the outside. Saline filled breast implants hold salt water (saline) within the shell. Silicone gel breast implants hold silicone gel within the shell. Breast implants come in various shapes, sizes, and shell types. Dr. Guerra uses breast implants from the two major manufacturers, Mentor Corporation and Allergan, Inc., in the United States.
Breast implants can be round, high profile, medium profile, low profile, elliptical, anatomic, and tear-drop shape. The size range for breast implants begins as low as 120 milliliters and goes up to 800 milliliters. The average sizes used in our practice is about 360 milliliters to 425 milliliters, but this varies from year to year. The outer breast implant shells come in two varieties, smooth and textured. All of these different possibilities when choosing your breast implants must be taken into consideration and it takes considerable experience to make the proper choice for each patient.
For breast implants to feel as natural as possible, they must be relatively soft. Silicone gel breast implants are much softer than saline breast implants and are both FDA approved.
What if your breast implant ruptures?
If your saline breast implant ruptures, the implant will deflate and causing the breast to change in size and shape. The leaking saline solution in your implant will be absorbed by your body without posing health risks. Breast surgery will be required to remove the silicone shell at some point. A new breast implant can be inserted at the same time for a breast implant exchange.
If your silicone breast implant ruptures, you may not notice any changes at any time. There is currently no evidence that leaking silicone breast implant gel causes serious, long-term health problems. A ruptured silicone gel breast implant may eventually cause breast pain or changes in the shape of the breast. If this happens, breast surgery to remove or replace the ruptured implant will be required. A new silicone gel breast implant can likely be inserted at the same time.
The Food and Drug Administration recommends monitoring silicone gel breast implants with routine MRI scans every two years, starting three years after the initial implant surgery. An MRI scan may detect a breast implant rupture when there are no signs or symptoms.
