How Under-the-Muscle Implants Work

In under-the-muscle breast augmentation, the implants are placed beneath the pectoral muscles rather than directly behind the breast tissue. This technique is commonly preferred by women who desire a more natural appearance because the muscle adds an additional layer of coverage over the implant, making it less noticeable, especially in the upper chest area. It also helps to camouflage the implant edges and can give the breasts a softer, more feminine look.

This method is especially popular for women with thinner breast tissue or those who want a fuller, rounder appearance without the risk of visible implant outlines. However, the muscles used for the implant placement can cause a temporary but significant effect on how the implants appear during the recovery process.

The Importance of “Dropping and Fluffing”

After your breast augmentation surgery, you’ll notice that your implants sit quite high on your chest. This is due to muscle spasm. The pectoral muscles are tightened from the placement of the implants and can push the implants upward in the immediate aftermath of surgery. In the first few weeks following the procedure, these spasms often prevent the implants from naturally settling into their final position.

The term “dropping and fluffing” refers to two key stages in the healing process that allow the implants to soften and lower into their final, more natural position:

  • Dropping: This phase occurs when the implants begin to shift downward after being initially pulled high on the chest. The muscle starts to relax, and the implants descend into a more natural-looking position.
  • Fluffing: During this phase, the implants begin to soften and become rounder, taking on a more full, voluminous appearance. This stage can take several months, as the breast tissue and muscle slowly stretch and adjust to the implants.

The process of dropping and fluffing can take anywhere from 2 - 4 months or even longer in some cases, depending on the individual’s body type, implant size, and healing process. It’s important to have patience during this time as the final results may not be immediately apparent.

Why Patience Is Essential

It’s easy to feel concerned when your implants appear higher than expected or firmer in the initial weeks, but this is completely normal. The muscle is still adjusting, and the implants will settle into a more natural position as time goes on. Rushing the process or being overly concerned about the early results can be discouraging.

Furthermore, maintaining a healthy recovery routine—such as following your surgeon’s post-operative instructions regarding activity restrictions, massage techniques, and wearing supportive bras—can help speed up the process and ensure optimal results.
