“What if my bottom looks too flat after a spiral thigh lift?”

They also preserve and purify the fat collected during liposuction, which is then reinjected into the buttocks. “Fat is liquid gold,” said Dr. Guerra in Episode 5 of The Slice Is Right Podcast. “We use it to restore volume where needed, usually with 500 to 600 cc’s.”

This procedure ultimately corrects the limitations of older techniques. Here’s what patients can expect:

  1. Lift and Reshape the Buttocks and Thighs

While the original method helped reshape the thighs to some degree, it often left behind the very issues patients sought to correct. Many still dealt with sagging around the lower buttocks and inner thighs, especially after significant weight loss. The tissue shifted but didn’t “hold” where it needed to. In practice, it focused too much on pulling skin around the body and not enough on lifting it. This limitation became too consistent to ignore, which led to changes in technique.

Dr. Aldo Guerra explained, “We’re lifting above the underwear line and in the thigh creases. We’re hiding the scars really nicely.” Another colleague, Dr. Scott Ogley added, “You don’t have to have the old vertical scar right down the middle of your thigh anymore.”

Skin removal alone can flatten the buttocks, especially in patients who have already lost volume during weight loss. To address this concern, Dr. Guerra and Dr. Dang collect and purify the fat removed during liposuction. They then reinject that fat into the buttocks to restore natural shape.

  1. Drain-Free Healing

Older thigh lift techniques required surgical drains to prevent fluid buildup. The modified method forgoes this step entirely. Dr. Guerra and Dr. Ogley perform a liposuction before removing the skin to separate the tissue layers and loosen the surrounding structures. This step reduces tension during excision, limits fluid buildup during recovery, and eliminates the need for a drain.

  1. Less Discomfort Than Traditional Thigh Lifts

Earlier thigh lift techniques involved attaching skin to the pelvic fascia, which increased tension and made the procedure difficult to tolerate during recovery. The modified spiral thigh lift avoids that outcome, as Dr. Guerra and Dr. Dang preserve the soft tissue beneath the skin and avoid anchoring the closure to deep structures.

This change reduces tissue strain, lowers nerve irritation, and improves post-operative mobility. Dr. Scott Ogley from the episode on The Slice is Right Podcast stated, “I don’t tend to have big issues with pain control with this surgery. Patients tend to be very happy. The swelling is minimal.”

  1. Long-Lasting Results

Some patients have asked whether the sagging will return. “It won’t come back because we’re removing so much skin,” Dr. Guerra answers. “Unless there’s a significant amount of weight change again or some other confounding factor,” the sagging will not return, Dr. Ogley added.

However, keep in mind that this procedure will not stop the aging process. Dr. Guerra compared it to a facelift, where the outcome lasts, but changes will develop over time. As an example, he said, “If you get a facelift in your 60s, you might need a touch-up once you’re 75. The same idea applies to a modified spiral thigh lift, meaning you might need a little touch-up later on.”
