A very common question I get is,”What is the difference between an open vs. closed rhinoplasty? Which is better? Which do you do?” To simplify, the only difference is that an “open” rhinoplasty involves an external incision on the columella, or the middle part of your nose, in between the nostrils. This is the more traditional method and the one that most rhinoplasty surgeons use.

In the “closed” technique, all the incisions are on the inside of the nose. Now, to determine which is better, that’s been a long debate amongst plastic surgeons for many years. There are great surgeons who do an open technique and great surgeons who do it with the closed technique. Some do both and base it on what the patient needs. At the end of the day, both techniques can deliver phenomenal results.

Case Study: Open Rhinoplasty on a 25-year-old patient.

A 25 y/o caucasian female who underwent a primary open rhinoplasty with Double Board-Certified Plastic Surgeon Jonathan Dang, MD. She had a dorsal hump reduction, tip elevation, and refinement using cartilage grafts from her own septum using an open technique. Surgery time was three hours.

Dr. Dang primarily uses the open rhinoplasty technique for most of his cases.

The most important thing to remember is that the surgeon will choose the technique that he/she is most comfortable with. Asking about technique is certainly a very important thing to discuss during your consultation, as this enables a more in-depth conversation between you and your surgeon and establishes a better rapport. For me personally, I was trained on the open technique, and it is the one I use primarily. It gives me the most visualization during surgery, and I especially like it when I need to do a lot of work on the tip of the nose.

I do like a closed approach whenever I’m addressing a deviated septum or performing a conservative hump reduction on the dorsum or top of the nose bridge. In terms of which is better, both have their advantages and disadvantages. The open technique is easier to teach and technically more straightforward. There is an external scar for the first few weeks, but this fades with time. The swelling at the tip of the nose will also take longer to resolve with the open technique.

On the contrary, the closed technique is more technically advanced, but there are no external incisions, and the postoperative swelling is much less. Remember, technique is the icing on the cake, but long-term results and safety will always be the number one priority when choosing your surgeon.
