Gravity and Cellulite Treatment

With the progression of time and the effects of gravity, the skin of the buttocks and thighs appears more elastic, loose, and develops a cellulite pattern which can become aesthetically distressing for our patients. Moreover, treatment for both cellulite and excessive skin of the buttocks and thighs that is effective and produces long standing results has been historically difficult to find in the past. We find that many patients are frustrated with the lack of options to restore a more youthful gluteal thigh physique.

Modified Spiral Thigh Lift Details

After the markings are carefully placed on the patient, surgery begins with Vaser assisted liposuction or Power assisted liposuction of the areas to be removed. Liposuction with this operation is aggressive and serves two main purposes. The first being the reduction of the fatty bulges under the skin to be removed. The second is the preservation of the lymphatic system, veins, and underlying nerves supplying the targeted areas. The advantages of body contouring surgery performed in this fashion include:

  • Preservation of lymphatics leads to less swelling.
  • Preserving the lymphatics nullifies the need for drains.
  • Preservation of lymphatics and veins lessens lower leg edema.
  • Preserving the local nerves leads to better skin sensation after recovery.
  • Preserved nerves in the surgical field leads to less overall pain during recovery.

We have observed that lipoatrophy deflation of the buttocks is a contributor to both cellulite and a flat appearing gluteal area. The fat removed with liposuction is therefore not discarded, but instead is used for lipofilling of deficient or deflated areas in the glutes. Appropriate volume restoration of these deflated areas, typically incorporating between 300 to 500 milliliters per side of your own fat, creates the most aesthetically pleasing results for patients and more effectively targets cellulite reduction compared to only performing a lift of the saggy buttock and outer thigh skin.

WHAT ARE ALL THE MODIFICATIONS OF THE MODIFIED SPIRAL THIGH LIFT?

A Modified Spiral Thigh Lift incorporates multiple previously described plastic surgery operations into a single and effective body contouring procedure that specifically targets aesthetic improvements to the gluteal thigh anatomy. For over a decade, Guerra Plastic Surgery Center, has worked to refine the Modified Spiral Thigh Lift with the goals of reducing the amount of cellulite while simultaneously lifting sagging and deflated skin in the gluteal thigh region. We are going to discuss all the modifications we have made to create the Modified Spiral Thigh Lift and provide a rationale for the use of each modification. However, we must first learn more about the components of the Modified Spiral Thigh Lift.

WHAT ARE THE COMPONENTS OF THE MODIFIED SPIRAL THIGH LIFT?

  • Reorientation of the incisions to create a more powerful vertical lift.
  • A Direct Gluteal Lift without an autologous flap.
  • An Outer Thigh Lift similar to that used with Lower Body Lift.
  • Suction lipectomy for volume reduction of unwanted lipodystrophy.
  • Volumizing and reshaping fat transfer to the buttocks.
  • A Medial Thigh lift hidden into the inner thigh crease.
  • A posterior Gluteal-Thigh Lift hidden into the gluteal fold.

HOW DOES OUR TECHNIQUE COMPARE TO THE ORIGINAL SPIRAL THIGH LIFT?

The original spiral thigh lift described in 2008 by other surgeons, incorporated a continuous and lengthy incision beginning over the patient’s sacrum which then darted forward to the hips and abdomen before dipping into the anterior inner thigh crease, which then further curved in proximity to the perineum, and finally ending in the posterior gluteal thigh crease. You will notice the description is consistent with a spiral, hence the name of this technique is derived from the incision configuration. The technique utilized drains to remove inflammatory postoperative fluids and called the rotation of a dermal fat flap to augment the associated deflated buttocks.

WHAT DID WE CHANGE FIRST WITH THE MODIFIED SPIRAL THIGH LIFT?

We observed that utilizing a continuous incision which gently slopes downwards as it meanders forward from the sacrum towards the abdomen and into the thighs actually reduces the effective vertical lift the plastic surgeon can achieve. Our very first modification, therefore, was to reorient the upper incision to create a more vertical and effective vector of lift in the gluteal and outer thigh region. Furthermore, this approach nullifies the continuous incision which creates some aesthetic deficiencies without any positive gains.

DOES YOUR MODIFICATION USE LIPOSUCTION AND FAT TRANSFER OR A DERMAL-FAT FLAP?

We eliminated the dermal-fat flap because it has limited reach and does not adequately fill the most deficient hip dips and gluteal depressions. The flap is too short and simply cannot reach the most deficient areas. Additionally, fat transfer was observed to have better reach, be far more efficient at filling the cellulitic depressions and contour problems, and introduce rejuvenating adipose stem cells to the glutes. Liposuction is able to reduce the fatty collections under the skin to be removed while simultaneously preserving the underlying lymphatic system, veins, and nerves supplying the donor areas.

A summary of advantages of the liposuction-fat transfer approach in the Modified Spiral Thigh LIft include:

  • Results in less swelling, lessens lower leg edema, and abolishes the need for drains.
  • Saving of nerves leads to better skin sensation and less overall pain with surgery.
  • Fat transfer more effectively fills depressions, cellulite, and leads to rounder glutes.
  • Gluteal fat grafting creates a more ideal waist to hip ratio.
  • Adipose stem cells contribute growth factors and improve skin quality.

HOW DOES THE MODIFIED SPIRAL THIGH LIFT REDUCE CELLULITE AND RECONTOUR THE GLUTES?

Clinical observations show that major weight loss and age-related lipoatrophy of the buttocks are dominant contributors to cellulite, saggy thighs, and a flat appearing gluteal anatomy. These findings create several cosmetic deficiencies which are very distressing to our patients, but can be addressed with the Modified Spiral Thigh Lift. Gluteal-hip dip volume restoration utilizing your own fat cells, injecting around 300 to 500 cubic centimeters per side, combined with excess skin removal in the designated anatomical areas, can create an aesthetically pleasing result for patients.
