Phone Tracking and Recording, SMS Texting
We use tracking phone numbers which redirect calls to our office, provided by services such as CallRail. These tracking phone numbers may replace the phone number that appears on our website. By calling a phone number displayed on our website, you are providing some call statistics such as your phone number, date and time, and call duration that are collected.
Our office records all incoming and outgoing phone calls, regardless of phone number, for our records.
When you dial our phone number, the call may connect with our leads partner Elite Med Listings. Any information shared with them is stored for future contact with you only. This information is shared to us, and no other party.
When you send a SMS Text to our number, it may be handled by Elite Med Listings, and shared to us. This information is not shared to any other party.
Any messages you receive from our SMS Text number are either replies to contact from you (via Contact Form, email, phone call, SMS text), or information you subscribed to such as appointment reminders. We do not engage in mass SMS marketing.
Do Not Track Requests
Do Not Track is a preference you can set on your browser to inform websites that you do not want to be tracked. We do not support Do Not Track (“DNT”). You can either enable or disable Do Not Track by visiting the Preferences or Settings page of your browser.
Data Collection by using this website
Third Party Partners

Contact forms
We collect contact information such as name, email address, phone number, IP address, and website url where the form is located. The “schedule a consultation” form also collects information such as your mailing address and procedure you are interested in. We use this information to contact you in the case that you have specified via the “contact form” on our web site that you are interested in making an initial consultation at our office. This information may go to our leads partner Elite Med Listings and be reviewed for first contact with you in order to respond directly to your questions or comments. We may also file your comments to improve the site, or review and discard the information.
Information in the form is also collected by P5 marketing for leads generation purposes.
Analytics and Cookies
We use Google Analytics, Google AdWords, and Facebook tracking pixel to track user activity and engagement to the website. This data does not allow us to identify data pertaining to individual users. This data may be shared with our partnered marketing companies to evaluate performance of the website and various promotions. We do not share this data for marketing products or services that are not offered by our office.

Tracking Pixel
We embed a tracking pixel on every page of this website to track your activity while on our website. Website activity including contact form data and pages visited is collected by P5 marketing and subject to their privacy policy. This technology enables anonymous visitor identification. They share lead data to us such as name, addresses, phone number, email address, and IP address. We use this information to contact potential patients who have not initiated contact with us.
As per their privacy policy, they do not sell your information to other parties.

Newsletter Signup
You may sign up to our newsletter by clicking on the Newsletter Signup button and submitting the form located on the bottom of every webpage on this site. We use MailChimp to handle all aspects of sending newsletters, including membership signup and unsubscribe. Data collected by the MailChimp forms are only used for the purpose of sending newsletters and promotions by our office. Data collected by the MailChimp forms are not shared with any third parties.
You must click on the checkbox indicated as “Email” under Marketing Permissions to give us consent to receive our newsletters.

Eventbrite Events
We use Eventbrite to manage event promotion and registration. Data collected by Eventbrite is used by our office to communicate with attendees only. Data collected by Eventbrite is not shared with any third party.

YouTube Videos
We use YouTube to display videos posted by our office. YouTube collects non-identifiable statistics such as how many views a video has, or how long the average view is for each video. By clicking on an embedded video on our website you are consenting to have this data collected by YouTube and are bound by YouTube’s privacy policy.
Facebook embeds
Some pages may have content embedded from Facebook. By clicking on any buttons inside a Facebook embed, you are consenting to Facebook collecting your data, and are bound to Facebook’s privacy policy.
